The Social Service/Discharge Planning Department helps patients and families plan post-hospital care. This includes arranging for home care or transfers to Skilled Nursing Facilities. The social worker works closely with interdisciplinary teams to develop a holistic plan of care for the patient. Referrals for all Social Service/Discharge Planning Services are accepted from our medical staff, hospital personnel, patients, families, and other health care professionals as appropriate.
