Illness and hospitals can cause anxiety, depression, anger or loneliness. Our medical social worker can help refer you to appropriare community resources.

Referrals for all Social Service/Discharge Planning Services are accepted from our medical staff, hospital personnel, patients, families, and other health care professionals as appropriate. Referrals may be made in person, by telephone or written contact. Treatment modalities of psychosocial services are available including crisis intervention, short-term counseling related to health care, discharge planning and referrals to appropriate community agencies. The social worker works closely with the interdisciplinary teams to develop a holistic plan of care for the patient. All team members working together with the patient/family successfully execute the plan.
