The Support Health Services assists the physicians to manage their patients with chronic diseases. It offers management in diabetes, women’s health, hepatitis B and immunizations.

Diabetes

• Provides individual diabetes self-management education.
• In collaboration with the primary care provider, assist the patient to attain better diabetes control via medical and lifestyle management.

Women’s Health

• Provides free annual gynecological exams, pap smears, breast exams, mammograms, and other women’s needs.

Hepatitis B

• Provides free Hepatitis B screening, low cost hepatitis B vaccines and assists the patient in applying for medication assistance if treatment is indicated
